Here, You’ll Learn How to Access Professional Brain Injury Treatment Services
Anyone can make a referral to CHIRS for Brain Injury Support Services. We will require the following information before bringing the request for service to our Intake Committee.
Once all the information is received, we will contact you to set up a screening interview with an Intake Facilitator who will then bring the request to the CHIRS Intake Committee for review. A letter will be sent to the individual and/or referring agent with the decision of the Intake Committee. If you and/or the referring agent are dissatisfied with the decision of the Intake Committee, we will provide you with information for appeal.
