The Application Process:

Anyone can make a referral to CHIRS.  We will require the following information before bringing the request for service to our Intake Committee:

  • A completed ABI Community Profile and relevant brain injury medical and consult reports must be included and submitted to the Toronto ABI Network by fax.   

    The Toronto ABI Network offers a single point referral service to publicly funded member agencies.  CHIRS is a member agency of the Toronto ABI Network.

    CHIRS services are offered on a subsidized and fee-for-service service basis.  All applicants requesting subsidy will be required to complete an income disclosure as part of the application process.

  • Individuals accessing services with third party funding should also complete the above application form, but must forward it with the relevant medical documentation directly to CHIRS by fax, mail or email.

Once all the information is received, we will contact you to set up a screening interview with an Intake Facilitator who will then bring the request to the CHIRS Intake Committee for review.  A letter will be sent to the individual and/or referring agent with the decision of the Intake Committee.   If you and/or the referring agent are dissatisfied with the decision of the Intake Committee, we will provide you with information for appeal.
